Abstract Subcutaneous emphysema is a common entity following facial trauma. However, pneumomediastinum secondary to isolated fractures rare and potentially life-threatening complication. We present case of 26-year-old female that presented with sudden onset left sided swelling over the periorbital zygomaticomaxillary region some bruising overlying maxilla. In addition palpable crepitus was temporal supraclavicular regions blunt There no associated difficulty breathing or dysphagia it unclear whether she had blown her nose earlier day. Computed tomography confirmed diagnosis extension pneumomediastinum. conclusion, complication Prophylactic antibiotics should be considered reduce risk mediastinitis blowing avoided. Due lack clear guidelines when managing this condition, sound clinical judgement high index suspicion recommended.
